摘要：

Specific examples of crashes and near midair collisions are used to identify existing and potential human error sources. System-induced human error caused by radar and information-processing limitations, inadequate communication capabilities, and Federal Aviation Administration policy decisions are discussed. An overall reconsideration of human error analysis and prevention is proposed.
