The blood flow and tissue phase images of dynamic scintigraphy may reveal unsuspected vascular lesions. Following nephrectomy, vascular activity in the renal bed usually reflects mesenteric vasculature. It has been speculated that metastases from renal cell carcinoma are insufficiently vascularized to produce a tumor blush. The authors report a case in which vascular activity in the renal bed on dynamic skeletal scintigraphy led to the discovery of a metastasis or local recurrence of renal cell carcinoma. A literature search has not revealed a similar case.
