ENDOSCOPIC retrograde cholangiopancreatography as a diagnostic procedure in patients with persistent, unexplained upper abdominal pain has disclosed numerous cases of pancreatic ducts with areas of constriction and distal dilatation. These changes are often associated with pancreatic carcinoma, but they may also result from inflammatory or other benign conditions. Indeed, the task of differentiating a duct deformity caused by an early, possibly curable cancer from a benign lesion currently poses a most difficult diagnostic and therapeutic dilemma. This report describes a patient in whom endoscopic retrograde cholangiopancreatography revealed major duct obstruction that appeared typical of pancreatic carcinoma.
