A clinical replication series of 38 patients meeting Research Diagnostic Criteria for probable (n=6) or definite (n=32) endogenous depression (ED) were treated in 16 week, 20 session protocol with cognitive behavior therapy (CBT). Nearly 75% (28/38) of patients met response criteria at the end of the protocol, with robust reductions of symptomatic ratings (p's<0.0001). Contrary to our prediction, patients with reduced rapid eye movement sleep latency (a biological correlate of ED) generally responded as well to CBT as patients with nonreduced values, with reduced REM latency patients showing more rapid improvement on several measures. In the outpatient setting, CBT's spectrum of efficacy clearly appears to extend to ED patients, including those with at least one objective indicator of biological dysfunction. © 1991 Association for Advancement of Behavior Therapy.
