No general standards have been established for creating or evaluating the scales used to rate clinical phenomena. The need for such standards became apparent when we evaluated the Karnofsky Performance Status Scale. We studied inter-observer variability in the use of the Karnofsky scale when two pairs of physicians scored 60 patients on the scale. Agreement was only 34% for one pair and 29% for the other. Major sources of observer variability appeared to be the lack of operational criteria to define the major elements of the scale and the nonexhaustive aggregation of the scale's constituent elements. Recognition of the importance of these omissions can improve the evaluation of other clinical scales and assist the development and testing of new scales. © 1979.
