摘要
Therapeutic immunoadsorption is the selective removal of immunoglobulins from patients in an extracorporeal circuit. The biochemical and structural properties of Staphylococcal protein A and its utility as an immobilized ligand for this application are reviewed. Utilizing it and an adsorption system that allows large quantities of IgG to be removed without the need for replacement fluids, patients with either autoimmune disease or those sensitized to alloantigens e.g. HLA antigens or factor VIII, have been treated. The immunological basis of each disease, the pathological role of antibody in the disease process, and the use of immunosuppressive therapies in the treatment of patients are discussed. A comparison of results collected during our studies and those obtained with plasmapheresis is offered where relevant.
