Seven years ago, a procedure of surgically staging cervical cancer patients was begun at the University of Miami. An important aspect of the surgical staging of cervical cancer, and increasingly of interest in other gynecologic cancers, is the area of the paraaortic lymph nodes. There is considerable variation in the method of determining the status of the paraaortic nodes. In some institutions the dissection is limited to either the right or left side, and there is great variation in the extent of the procedure. Many surgeons rely on palpation only, or selective biopsy. With these preceding facts in mind, we decided to relate our experience with 450 paraaortic dissections in a variety of gynecologic malignancies. A detailed discussion of the anatomy and the technique that we have found successful is presented. Operative complications, as well as gross and microscopic findings as they relate to the development of our operative technique will be discussed. © 1979.
