瓜氨酸、肠型脂肪酸结合蛋白、肠三叶因子联合检测对儿童危重症急性胃肠损伤的诊断价值

摘要
目的探讨瓜氨酸、肠型脂肪酸结合蛋白(IFABP)、肠三叶因子(ITF)联合检测对儿童危重症急性胃肠损伤(AGI)的诊断价值。方法选择2014年9月至2015年8月在我院儿科住院治疗的危重症患儿100例,按患儿是否合并AGI分为观察组(合并AGI)52例和对照组(未合并AGI)48例。比较两组患者的瓜氨酸、IFABP、ITF、C反应蛋白(CRP)及降钙素原(PCT)水平;并分析瓜氨酸、IFABP、ITF与CRP及PCT水平的相关性。结果观察组患者的瓜氨酸水平显著低于对照组[(15.08±8.84)μmol/L vs(25.07±12.05)μmol/L],而IFABP[(1 373.38±410.55)ng/L vs(938.89±503.84)ng/L]、ITF[(14.51±4.90)ng/m L vs(9.79±4.82)ng/m L]、CRP[(60.65±21.08)mg/L vs(28.12±10.13)mg/L]及PCT[(22.93±10.19)ng/m L vs(5.28±3.08)ng/m L]均显著高于对照组,差异均有显著统计学意义(P<0.01);瓜氨酸、IFABP及ITF三者联合检测对儿童危重症AGI的诊断价值最高,其中敏感度为98.08%,特异度为95.83%,准确度为97.00%。根据Pearson法分析显示,瓜氨酸与CRP及PCT均呈负相关(r=-0.661,-0.709;均P<0.05),而IFABP和ITF与CRP及PCT无明显相关性(r=0.108,0.201,0.097,0.113;均P>0.05)。结论瓜氨酸与IFABP和ITF三者联合检测对危重症AGI患儿的诊断具有较高价值,值得在临床诊断过程中推广应用。
