摘要：

通过文献学习,对美国、英国、加拿大、澳大利亚等国出院服务、出院计划和出院报告的研究和实践经验进行综述。指出改善我国出院服务质量是提高区域内医疗服务连续性和卫生资源利用效率、改善患者健康结果的重要策略,这个改变需要宏观系统的支持。
