重症肿瘤患者肠道功能评估与血清细菌内毒素、D-乳酸及二胺氧化酶变化的相关性研究

摘要：

目的研究重症肿瘤患者肠道功能评估与血清D-乳酸(DLC)、细菌内毒素(BT)及二胺氧化酶(DAO)变化的相关性,为临床诊断及治疗提供参考。方法回顾性分析华中科技大学同济黄州医院ICU2015年12月至2017年12月收治的老年肿瘤患者234例为研究对象,根据是否合并胃肠功能障碍分组,合并胃肠功能障碍70例为研究组,发生率29.9%,另164例为对照组。所有受检者于入科24 h内进行急性生理与慢性健康评分(APACHEⅡ评分),并统计病死率。应用联检试剂盒(酶法)检测DLC、BT及DAO变化,并分析重症肿瘤患者APACHEⅡ评分与上述指标的相关性。结果研究组DLC、BT、DAO分别为(24.3±16.5)mg/L、(11.4±9.7)U/L、(16.5±22.6)U/L,均高于对照组的(11.7±12.4)mg/L、(9.3±10.2)U/L、(6. 3±9. 6) U/L,差异均有统计学意义(均P <0.05);Spearman相关分析显示,患者APACHEⅡ评分与DLC、BT及DAO均呈正相关(r=0. 53,P<0.05;r=0.45,P <0.05;r=0.43,P=0.01)。结论重症肿瘤胃肠功能障碍患者DLC、BT及DAO均明显升高,且与APACHEⅡ评分呈正相关,值得临床注意。
