呼吸机相关性肺炎的病原学诊断及治疗新进展

摘要
呼吸机相关性肺炎(VAP)是气管插管机械通气患者的常见病、多发病,其发病率、病死率均较高。小儿VAP的发病率将随着呼吸机的广泛应用而显著增加,是小儿死亡的常见原因之一。由于VAP诊断标准尚未完全规范化,故其漏诊率和误诊率较高。引起VAP的主要病原是耐药革兰阴性杆菌。治疗VAP关键在于合理使用抗生素,要求准确的病原学诊断及药敏试验。近年,纤维支气管镜技术因其具有可靠的诊断及特异的治疗效果已广泛应用于临床,是一项具有广阔前景的技术,对VAP的诊治及降低患者病死率具有极其重要的临床意义。
