降钙素原指导脓毒症患者抗生素应用的荟萃分析

摘要
目的 探讨降钙素原(procalcitonin,PCT)指导下脓毒症患者抗生素应用的临床效价。方法 以PCT、脓毒症、重症感染、抗生素、抗感染治疗等为检索词,检索万方医学数据库、中国知网、中国生物医学文献数据库,以procalcitonin、sepsis、antibiotics等为检索词检索Pubmed、ISI WOK数据库,共收集了相关文献475篇,通过Stata数据分析系统、Revman 5.2软件对合格的临床试验进行Meta分析。结果 共检索到相关文献475篇,排除非随机对照和队列研究文献349篇,排除其他不符合本研究要求文献119篇,纳入本研究Meta分析文献共7篇,总样本量达1520例。对抗生素应用时间、28 d死亡率、入住ICU时间、总住院时间通过GRADE系统进行评价,采用Revman 5.2软件对数据进行Meta分析。结果发现,PCT指导下对脓毒症患者应用抗生素治疗时,其抗生素应用时间和总住院时间与对照组比较差异有统计学意义,而28d死亡率及入住ICU时间与对照组比较差异无统计学意义。结论 对于脓毒症患者,在PCT指导下应用抗生素,与传统经验抗感染治疗比较,能使其应用抗生素时间明显缩短而不产生其他不良后果,并且缩短了患者的总住院时间,从而使整个临床治疗效果获益,所以PCT指导脓毒症患者抗感染治疗是可行而且安全的。
