早期血浆二胺氧化酶D-乳酸与急性胃肠损伤的相关性分析

摘要：

目的探讨早期血浆二胺氧化酶(DAO)、D-乳酸水平与急性胃肠损伤(AGI)严重程度的相关性,并获取其最佳截断值。方法选取2016年4月至2019年4月AGI患者151例,根据AGI诊断分级标准,分为Ⅰ级组、Ⅱ级组、Ⅲ级和Ⅳ级组,入组24h内采取静脉血,检测血浆DAO与D-乳酸的水平。结果四组患者的DAO和D-乳酸水平分别为(3.35±2.03) U/L、(25.83±13.76) U/L (Ⅰ级组),(14.92±17.33) U/L、(47.71±19.96) U/L (Ⅱ级组),(30.22±11.25) U/L、(59.23±14.08) U/L (Ⅲ级组),(53.84±24.54) U/L、(68.09±12.85) U/L (Ⅳ级组),四组间差异有统计学意义(P<0.05)。相关系数r为0.741 (95%CI 0.672～0.825,P<0.01)和0.681 (95%CI 0.589～0.758,P<0.01)。ROC曲线显示DAO和D-乳酸曲线下的面积分别为0.925 (95%CI 0.876～0.922;P<0.01)和0.841 (95%CI0.775～0.907;P<0.01)。DAO和D-乳酸的截断值分别为21.52U/L和48.16U/L,敏感性和特异性分别为0.923、0.863和0.91、0.712。结论早期血清DAO和D-乳酸水平与AGI等级呈正相关,对AGI严重程度的评估有一定的临床意义。
