颅脑损伤伴失血性休克脑血流代谢的研究

摘要
对颅脑损伤伴失血性休克时 7 5%氯化钠高渗液对脑血流代谢及全身血液动力学的影响进行前瞻性研究 .结果 :全身血液动力学参数在伤后 30min内 ,TBIHS1,TBIHS2 ,HS变化最显著 ,135min时HS恢复伤前水平 ,TBIHS1较TBIHS2 快速恢复至伤前水平下限 ,TBIHS1,TBIHS2 在伤后 30min内其SjVO2 均下降至脑缺血缺氧临界点 ,以后各组逐渐上升 ;肺组织含水量测定各组无明显增加 ;健侧脑组织含水量TBIHS2 增多 .结论 :颅脑损伤本身造成全身血液动力学及血流代谢改变 ,失血性休克加重了这一病理生理变化 ,高渗晶体液对纠正失血性休克有较好疗效而不增加脑含水量 ,纠正全身血液动力学需使其恢复至正常值下限即能满足脑血流代谢最大恢复值 .
