恩替卡韦抗病毒患者乙型肝炎病毒逆转录区准种动态演变及其临床意义分析

摘要
目的探讨恩替卡韦(ETV)初治应答不佳的慢性乙型病毒性肝炎(CHB)患者体内乙型肝炎病毒逆转录区(HBV RT)准种动态变化规律及其临床意义。方法收集5例2014年11月~2017年12月期间在我院住院及门诊应用ETV初始治疗(0.5 mg/d)应答不佳的患者资料,留取各患者治疗过程中基线、4周、12周、24周、36周和48周血清标本冻存于-70℃备用,并检查生化指标、血清学指标、HBV DNA水平等。提取HBV DNA,PCR法扩增HBV RT区,HBV-DNA≥1E+04 IU/mL的模板直接扩增,HBV DNA<1E+04 IU/mL的模板,应用巢式PCR扩增。回收目的片段并进行T-A克隆,每个标本挑取20~30个克隆,取鉴定为阳性的克隆测序,采用ClustalX 1.81软件和NCBI BLAST工具,将HBV序列进行比对,分析其氨基酸替换形式及准种分布情况。结果本实验5例ETV应答不佳患者治疗过程中共检出7种病毒株变异形式,分别是rtM204I、rtM204IV、rtT184L、rtM204V+rtL180、rtM204I+rtL180M、rtM204I+rtL180M+rtT184L、rtM204V+rtL180M+rtT184L。各种病毒株所占比例在不断发生变化,抗病毒之前野生株是绝对优势株,出现病毒学突破时,耐药突变株成为种群中优势株。结论在ETV抗病毒药物压力下,CHB患者HBV RT区准种分布处于动态变化中,准种变化与ETV抗病毒药物敏感性以及耐药性密切相关。rtM204I/V+rtL180M+rtT184L是与ETV耐药相关的主要变异模式。
