CT在乳腺肿瘤鉴别诊断中的应用

摘要
目的 分析乳腺良、恶性肿瘤的CT表现和特征 ,探讨增强前后CT值变化 (ΔCT)与肿瘤性质和大小及转移淋巴结的关系。方法 分析 1996年 7月~ 2 0 0 0年 4月经增强CT检查且具有病理结果的 81例乳腺肿瘤病人共 93个病灶 ,其中乳腺癌 36个 ,良性病变 5 7个。结果 良、恶性组平扫CT平均值分别为 2 1 2 4± 7 2 4HU和 2 5 6 5± 9 43HU (P >0 0 5 ) ,增强后CT值分别为 16 86± 17 77H和 35 36± 15 0 7HU(P <0 0 1) ;良、恶性组肿瘤边缘有毛刺或分叶状分别为6 3 89% (2 3/ 36 )和 15 79% (9/ 5 7) (P <0 0 1)。 <1cm的非钙化性良性病灶ΔCT值均 <30HU ,40 % <1cm的乳腺癌△CT值 >30HU。CT对转移淋巴结的总检出率为 5 3 3% (8/ 15 ) ,对N2 转移淋巴结的检出率为 85 71% (6 / 7)。结论 CT在乳腺疾病的鉴别诊断和乳腺癌的术前分期中具有一定的临床价值。
