维持性血液透析患者炎症-营养不良状态与血小板数量的关系

摘要
目的观察维持性血液透析(maintenance hemodialysis,MHD)患者炎症-营养不良状态与血小板数量的关系。方法选取首都医科大学附属北京同仁医院血液透析中心稳定的MHD患者68例,记录患者一般情况,检测血常规、血生化,根据血小板数量将患者分为6组,应用SPSS17.0软件进行统计学分析,采用单因素方差分析比较各组之间一般情况,血液学指标之间的差异,采取Spearman相关和线性回归分析患者血小板数量与患者一般情况及相关血液学指标的关系。结果 6组患者之间透析龄、血尿素氮、白蛋白、总胆固醇、低密度脂蛋白胆固醇、高敏C反应蛋白,白细胞、血红蛋白统计学差异显著(P<0.05),Spearman相关和线性回归分析显示血小板数量与高敏C反应蛋白(β=0.208,P<0.05)、低密度脂蛋白胆固醇(β=1.143,P<0.05)、白细胞数量(β=0.631,P<0.05)呈正相关,与血浆白蛋白(β=-0.320,P<0.05)、血红蛋白(β=-0.271,P<0.05)呈负相关。结论在MHD患者,炎症、营养不良状态与血小板数量增多有关,这可能是其增加血液透析患者心血管事件的机制之一。
