1Hz低频重复经颅磁刺激对脑卒中后偏瘫上肢痉挛及运动功能作用的meta分析

摘要
目的:系统评价低频重复经颅磁刺激(repetitive transcranial magnetic stimulation,rTMS)对脑卒中患者偏瘫上肢痉挛及肢体运动功能的疗效。方法:计算机检索PubMed数据库、Web of science、Medline、Cochrane Central Register of Controlled Trials(CENTRAL)、EMBASE、中国知识资源总库(China National Knowledge Infrastructure,CNKI)、中国生物医学文献数据库(China Biology Medicine disc,CBM disc)、维普中文科技期刊全文数据库(VIP Database for Chinese Technical Periodical)、万方数据库(Wanfang Data)。文献检索起止时限均为从建库到2016年10月。对不同文献分别按照Cochrane系统评价员手册推荐方法,及使用纽卡斯尔-渥太华量表(the Newcastle Ottawa Scale,NOS)对纳入研究的质量进行评估,提取有效数据,对符合质量要求的文献数据进行meta分析。采用Rev Man 5.3软件对最终纳入的文献数据进行统计学分析。结果:最终纳入7篇文献206例患者,定量分析5项研究,其余作描述性研究。meta分析2个结局指标改良Ashworth指数(Modified Ashworth Scale,MAS)、Fugl-Meyer运动量表评分(Fugl-Meyer Motor Scale,FMA),治疗的前后结果对照显示,rTMS对脑卒中患者指屈肌痉挛MAS评分的影响,其差异有显著性意义[SMD=0.77,95%CI(0.15,1.38)],对患者腕屈肌痉挛MAS评分的影响,其差异有显著性意义[SMD=0.83,95%CI(0.33,1.33)],对患者运动功能的治疗效果评价的FMA运动评分,其差异有显著性意义[SMD=-0.53,95%CI(-0.79,0.26)]。结论:现有研究显示,重复经颅磁刺激能改善脑卒中患者的上肢肌痉挛和提高肢体运动功能。鉴于纳入文献质量不高,仍需进一步开展多中心、大样本的高质量临床双盲随机对照试验。
