rTMS及MECT在不同时间点对抑郁症疗效和BDNF的影响及相关性研究

目的 探讨重复经颅磁刺激(rTMS)及无抽搐电休克治疗(MECT)在不同时间点对抑郁症疗效和血清脑源性神经营养因子(BDNF)的影响及其相关性研究。方法 将抑郁症患者分成rTMS组、MECT组和药物组。治疗前及治疗2、4、12周后分别评定3组患者的汉密尔顿抑郁量表(HAMD)评分,测定血清BDNF水平并分析其相关性。结果 治疗2周后,rTMS组和MECT组患者的HAMD评分低于药物组,差异均有统计学意义(P<0.01),rTMS组与MECT组的差异无统计学意义(P>0.05)。治疗2周和4周后rTMS组及MECT组患者的血清BDNF水平高于药物组,差异均有统计学意义(P<0.05),而rTMS组与MECT组的差异无统计学意义(P>0.05)。治疗4周后患者的血清BDNF水平与其HAMD评分呈正相关(r=0.407,P<0.05)。结论 单纯抗抑郁药物治疗2周后对抑郁症状的改善及血清BDNF水平升高的效果不及抗抑郁药物加rTMS、MECT治疗。
