应用脂肪衰减参数评价非酒精性脂肪肝与中医证型关系

摘要
目的:探讨脂肪衰减参数(FAI值)在非酒精性脂肪肝(NAFLD)的诊断价值及中医证型临床应用。方法:收集2014年2月至2015年2月福建省第二人民医院就诊的150例NAFLD患者。采用瞬时弹性成像技术(Fibro Touch)检测FAI值,彩超诊断脂肪肝,并行中医辨证分型。采用ROC曲线方法评价FAI的诊断准确性,并进行相关分析。结果:NAFLD痰瘀互结证患者FAI值最高,其次为痰湿内阻证、湿热蕴结证、肝郁脾虚证、肝郁气滞证(F=59.054,P<0.001)。FAI值与脂肪肝程度呈正相关(r=0.718,P<0.001)。组间比较,差异有统计意义(F=93.663,P<0.001)。FAI值与BMI、TG呈正相关(分别r=0.568,P<0.001;r=0.287,P<0.001)。FAI值诊断中度脂肪肝ROC曲线下面积为0.895(95%CI:0.844~0.947)、重度脂肪肝为0.956(95%CI:0.917~0.995)。结论:评估FAI值有助于脂肪肝中医辨证,在脂肪肝分级诊断中准确性较高,Fibro Touch技术具有良好的发展前景。
