胰岛素强化治疗在腹部大手术后危重患者肠外营养期间的应用

摘要：

目的探讨胰岛素强化治疗在腹部大手术后危重患者完全胃肠外营养期间的应用价值。方法选择78例腹部大手术后行完全胃肠外营养(TPN)治疗1周(或以上)的非糖尿病危重患者(血糖≥11.1mmol/L)随机分为两组;胰岛素强化治疗组(IT组,血糖目标值4.4～6.1mmol/L)和传统治疗组(CT组,血糖目标值<11.1mmol/L),分别监测血糖水平,低血糖发生以及切口感染和愈合情况。结果治疗后IT组的血糖控制情况明显优于CT组(P<0.01);IT组的切口感染率、切口愈合障碍率均低于CT组;两组低血糖发生率无统计学意义。结论胰岛素强化治疗控制腹部大手术后TPN支持期间危重患者的血糖,与传统治疗相比,低血糖情况的发生并不增多,而且可以减少术后切口感染,切口愈合障碍等并发症,值得在临床中推广应用。
