上海社区医院老年脑梗死患者服用阿司匹林现况及缺血性脑卒中复发调查

摘要：

目的调查上海部分社区医院住院老年脑梗死患者二级预防中阿司匹林的应用现况,探讨阿司匹林应用不恰当的原因,并分析其对预防缺血性脑卒中复发的效果。方法采用现况调查方法,将上海10家社区医院的223例脑梗死患者分为正规服用阿司匹林组(n=98)与未正规服用阿司匹林组(n=125),分析从脑梗死初发到调查时应用阿司匹林的情况及其与缺血性脑卒中复发的相关性。结果未正规服用阿司匹林组有51例(40.80%)患者因出现可能阿司匹林不良反应停药或减量,就总调查人数而言,不良反应发生率占总调查人数的22.87%,42例(33.60%)从未被建议使用阿司匹林或只单用活血化淤类中药;未正规服用阿司匹林组中62例(49.60%)再发缺血性脑卒中,正规服用阿司匹林组中30例(30.61%)再发缺血性脑卒中,两组比较差异有统计学意义(P<0.01)。合并糖尿病的脑梗死患者复发率(38.04%)与未合并者复发率(25.19%)比较,差具有统计学意义(P<0.05)。Logistic回归分析糖尿病为老年缺血性脑卒中患者复发的独立危险因素(OR=1.876,95%可信区间为1.056～3.331)。结论上海部分社区医院存在阿司匹林应用不足的状况;老年脑梗死幸存者服用抗血小板药物阿司匹林可以减少缺血性脑卒中复发。
