超敏C反应蛋白水平与冠心病严重程度及预后的相关性研究

目的研究冠心病患者超敏C反应蛋白(hs-CRP)水平与冠脉病变严重程度的关系,探讨hs-CRP水平对冠心病患者心血管事件的预测作用。方法纳入经冠状动脉造影明确诊断为冠心病的住院患者667例。记录所有患者的临床资料,包括hs-CRP水平、心血管病危险因素及左室射血分数等。并对这些患者进行随访,记录终点事件发生情况。终点事件主要指不良心血管事件(MACCE),包括全因死亡、靶血管病变重建、随访期间发生的心肌梗死、脑卒中或者一过性脑缺血、需要住院治疗的不稳定性心绞痛和心功能衰竭。结果平均随访时间为(15±12)个月,所有患者根据hs-CRP水平分为3组,高hs-CRP水平组(≥4.90 mg/L),中hs-CRP水平组(1.43~4.90 mg/L)和低hs-CRP水平组(<1.43 mg/L)。随访期间共146例患者发生MACCE,MACCE组患者较无MACCE组的hs-CRP水平明显升高(P<0.05)。单因素及多因素COX回归分析hs-CRP是冠心病患者将来MACCE的独立预测因素〔RR=1.06,95%CI(1.025,1.109),P=0.0035〕。Kaplan-Meier生存曲线显示随着hs-CRP水平升高,患者的无事件生存率明显降低(log-rankχ2=11.56,P=0.003)。结论 hs-CRP水平与冠心病患者的冠状动脉病变严重程度相关并且可以预测冠心病患者的不良预后。
