双侧训练作为启动训练在脑卒中后重度偏瘫患者上肢康复训练中的临床应用

摘要
目的观察在常规训练的基础上以双侧训练为启动训练对脑卒中后重度偏瘫患者上肢运动功能和日常生活活动能力的影响。方法选取脑卒中后重度偏瘫患者45例, 采用随机数字表法将其分为常规组、单侧训练组和双侧训练组, 每组患者15例。3组患者均进行常规康复训练, 单侧训练组在常规训练的基础上以单侧(患侧训练)训练为启动活动, 双侧训练组在常规训练的基础上以双侧训练为启动活动。于治疗前和治疗4周后(治疗后)对3组患者的运动功能(FMA-UE)及日常生活活动能力(MBI)进行评定。结果治疗后, 3组患者FMA-UE和MBI评分较组内治疗前均显著改善, 差异均有统计学意义(P<0.05);且双侧训练组治疗后的FMA-UE和MBI评分分别为(35.73±5.12)分和(62.13±5.67)分, 均显著优于单侧训练组和常规训练组治疗后, 差异均有统计学意义(P<0.05)。结论在常规训练的基础上以双侧训练为启动活动可显著改善脑卒中后重度偏瘫患者的上肢运动功能和日常生活活动能力。
