摘要
本文概述TL-1及TNF-α在RA炎症反应中的近期研究成果。IL-1与TNF-α均由单核/巨核细胞产生,主要参与(1)激活血管内皮细胞、增加内皮细胞粘附分子表达、使中性粒细胞在滑膜腔内集聚;(2)刺激滑膜细胞和中性粒细胞产生前列腺素等炎性介质;(3)刺激软骨细胞和破骨细胞减少糖蛋白合成、增加糖蛋白降解。并产生胶原酶及其他中性蛋白酸类,释放骨钙等。从而导致RA滑膜、软骨与软骨下骨的破坏,并与RA的发热、贫血、急性期反应蛋白合成等密切相关,在RA炎症中处于中心地位。应用克隆的IL-1R、IL-lra以及基因重组鼠抗人TNF-α可变区与人免疫球蛋白恒定区嵌合抗体治疗RA等,已取得可喜成效。
