摘要：

药物治疗管理是专业药师对患者提供用药教育、咨询指导等一系列服务,从而提高用药依从性,预防用药错误,培训患者进行自我用药管理,以提高疗效的服务模式。其核心要素包括药物治疗回顾、个人药物记录、药物相关活动计划、干预和/或提出参考意见以及文档记录和随访。本文通过详细介绍其运作模式,梳理美国药物治疗管理的发展历程以及综述国内外针对药物治疗管理服务提供者、服务对象、服务过程、服务效果等方面的研究,以期为促进我国药学服务发展、建立药物治疗管理体系提供参考。
