老年溃疡性结肠炎患者生活质量评价研究

摘要：

目的探讨老年溃疡性结肠炎(UC)患者的生活质量及其影响因素。方法应用中文版炎症性肠病问卷(C-IBDQ), 对老年UC患者进行横断面调查, 评价其生活质量(QOL);通过相关分析和多元线性回归分析, 筛选出影响老年UC患者QOL的主要因素。结果 90例老年UC患者完成问卷, QOL总分为(175.7±37.6)分, 各维度评分均有下降, 以经济负担、全身症状及情感功能下降较为明显;多元线性回归分析提示, 对老年UC患者QOL有影响的因素为性别、病情、病程、疾病活动度及付费方式等;男性UC患者肠道症状得分明显高于女性患者〔(53.9±12.1)分比(49.1±10.5)分, P=0.049〕;病程<1年的患者的QOL明显低于病程1年以上者(P<0.05);重型UC患者的QOL明显低于轻型患者(P<0.05);活动期患者QOL明显低于缓解期患者(149.7±35.1比193.9±27.3;t=6.731, P=0.000);医疗费用主要由个人负担的患者QOL较费用大部分由医保支付的患者低, 二者在全身症状、社会功能及经济负担等3个维度差异有统计学意义(P<0.05)。结论老年UC患者QOL均有不同程度受损;影响老年UC患者QOL的因素有性别、病情、病程、疾病活动度及付费方式;经济负担对UC患者QOL有明显影响。
