大肠择期手术前行机械肠道准备的必要性探讨

目的通过检索和评价现有医学证据,指导大肠择期手术前肠道准备方案的选择。方法针对具体问题提出相关临床问题,检索、筛选并评价对近10年PubMed、MEDLINE、EMbase、Cochrane图书馆、Science和ACP网站有关大肠择期手术前准备的文献。结果共纳入17篇文献,其中系统评价2篇,Meta分析6篇,随机对照试验9篇。大多研究表明大肠择期手术前机械肠道准备无益处,不做机械肠道准备并不增加吻合口漏的发生率。相反,机械肠道准备还可能导致吻合口漏发生率和伤口并发症增加。据此,我们对该患者选择了不做机械肠道准备的一种创新性简化术前肠道准备方案,与我国临床实际相结合并收到预期效果。结论几乎有所系统评价和Meta分析均表明无证据支持大肠择期手术前机械肠道准备有益。而文中报道的华西医院简化术前肠道准备方案值得进一步研究。
