银杏叶提取物对血栓前状态相关指标影响的Meta分析

摘要
目的评价口服单剂量银杏叶提取物(GBE)对血栓前状态相关指标的影响。方法检索数据库包括:Medline、EMbase、Cochrane Library、中国生物医学文献数据库(CBM)、计算机检索中国期刊全文数据库(CNKI)、维普中文科技期刊全文数据库和万方数据库,检索时限为建库至2014年5月16日。检索中英文关于GBE的临床随机安慰剂对照试验。考察血栓前状态相关指标:血小板聚集、活化部分凝血活酶时间(APTT)、血浆凝血酶原时间(PT)、血浆纤维蛋白原、全血黏度,血液流速。采用Cochrane协作网提供的Rev Man 5.0软件进Meta分析。计量资料采用加权均数差(WMD)或标准化均数差(SMD)及其95%置信区间(CI)表示。各纳入研究结果间进行异质性检验,当各研究结果间有统计学同质性时(P>0.1,I2<50%),采用固定效应模型进行Meta分析,否则采用随机效应模型分析,并寻找其异质性来源,对可能导致异质性的因素行敏感性分析或亚组分析。结果纳入了10个国家的14个RCTs研究,共计440例成人受试者参加随机分组。结果与安慰剂组相比,GBE对部分血栓前状态相关指标的影响存在明显统计学差异,对二磷酸腺苷(ADP)诱导血小板聚集有明显抑制作用〔WMD-7.53%,95%CI(-14.27,-0.78),P=0.03〕,有显著的降低全血黏度的作用〔WMD-1.40 m Pa·s,95%CI(-1.91,-0.89),P<0.000 01〕,并且显著增加血液流速〔SMD 0.57,95%CI(0.17,0.96),P=0.005〕。但对以下指标的作用差异无统计学意义:APTT〔WMD-0.25 s,95%CI(-0.68,0.19),P=0.26〕、PT〔WMD-0.07 s,95%CI(-0.37,0.23),P=0.64〕、血浆纤维蛋白原〔WMD-4.94 mg/dl,95%CI(-23.02,13.15),P=0.59〕。结论 GBE具有明显抑制ADP诱导的血小板聚集、降低全血黏度和显著增加血流速度的作用,从而有效改善血栓前状态,对血栓形成具有抑制作用。
