糖尿病患者使用碘对比剂安全性循证评价

摘要：

对比剂肾病(CIN)已成为医院获得性肾病的第三大病因,糖尿病(DM)、对比剂类型和动脉途径给药是CIN的重要风险因素,在使用碘对比剂的DM和肾功能不全人群中,CIN的发生率甚高,影像与介入医师对此尚未得到足够重视。本文就DM患者使用碘对比剂的安全性、各种对比剂的肾毒性等进行了循证评价,单纯DM患者使用低渗非离子型造影剂相对安全;高风险患者静脉使用碘对比剂不会对患者肾功能产生永久性损害,DM肾病或DM合并肾功能不全者动脉注射低渗对比剂(LOCM)发生CIN的风险较大,此类患者使用等渗对比剂(IOCM)碘可沙醇发生的风险较低且经济有效。
