3DCRT治疗肝癌并门静脉癌栓的疗效评价

摘要：

目的:分析三维适形放疗技术治疗原发性肝癌并门静脉癌栓的疗效。方法:2001年10月-2006年10月,共收治25例原发性肝癌并门静脉癌栓患者,采用三维适形放疗技术治疗,分割剂量视病情定为2-5Gy/次,治疗10-30次,90%以上的等剂量线包绕靶区,靶区总剂量50-70Gy。观察肿瘤及癌栓的近期疗效,用Kaplan-Meier法进行生存分析,Cox比例风险模型作多因素分析。结果:原发灶肿瘤近期有效率60.0%,癌栓有效率为76%。平均生存时间为(26.5±9.5)个月,中位生存时间为(24.0±8.46)个月。多因素分析显示肿瘤分期、肝功Child-Pugh分级、卡氏评分是影响预后的主要因素(P<0.05)。结论:3DCRT具有疗效好,损伤小,易耐受,是治疗原发性肝癌并门静脉癌栓有效治疗方法。
