甲状旁腺素与醛固酮的相互作用及其对心血管疾病的影响

摘要
甲状旁腺素(PTH)与醛固酮的不恰当分泌增多与心血管系统疾病的发生发展密切相关。有证据表明PTH与醛固酮互相影响,二者相互作用可提高心血管疾病、代谢性骨病发生的风险。本文就二者关系及其对心血管疾病影响的发病机制进行综述。PTH可直接作用于肾上腺球状带或在肾素-血管紧张素-醛固酮系统介导下间接促进醛固酮分泌。甲状旁腺功能亢进症患者醛固酮水平升高是发生心血管疾病的主要原因之一。甲状旁腺切除术后醛固酮水平降低,心血管疾病发病率也随之下降。原发性醛固酮增多症的患者因钙丢失增多引起PTH分泌,在一定程度上加剧醛固酮分泌及对心血管系统损害,肾上腺切除术或盐皮质激素受体拮抗剂可能是缓解上述病变的治疗选择。尚需进一步研究证实二者相互作用关系的发生机制,从而为心血管系统疾病的诊断及治疗提供更多更好的选择。
