脑梗死患者血清MCP-1的变化及阿托伐他汀的干预作用

目的观察急性动脉粥样硬化性脑梗死患者血清中单核细胞趋化因子(MCP-1)水平及颈动脉粥样硬化斑块,研究MCP-1与缺血性卒中的关系,并探讨阿托伐他汀的抗炎作用。方法用ELISA法检测急性动脉粥样硬化性脑梗死患者79例(CI组)、短暂性脑缺血发作患者16例(TIA组)、正常对照组30例血清MCP-1水平,同时通过彩色多普勒超声检查颈动脉粥样硬化斑块,将有软斑块或血清低密度脂蛋白胆固醇(LDL-C)>2.6mmol/L的脑梗死患者分成A组(服用阿托伐他汀,n=21)和NA组(未用任何他汀类药物,n=23),治疗3个月后随访,观察两组血脂及血清MCP-1的变化。结果CI组及TIA组患者血清MCP-1水平较正常对照组显著升高(P<0.01)。治疗后A组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及血清MCP-1显著下降(P<0.01),NA组均无显著变化。治疗后A组与NA组血清MCP-1、TC及LDL-C比较差异有统计学意义(P<0.05)。结论MCP-1与缺血性卒中的发生有关,可能参与了脑梗死的免疫过程。阿托伐他汀可降低脑梗死患者TC、LDL-C及MCP-1水平,具有一定的抗炎作用。
