肌电学方法在面神经功能评价中的应用效果评价

摘要
目的 :探讨神经电图(ENo G)和表面肌电信号(sEMG)在评价面神经功能中的临床应用价值,为面神经功能评价提供客观的实验参数。方法:通过运用House-Brackmann(HB)分区评价系统和ENo G、sEMG,评价腮腺手术对面神经造成的医源性损伤。选择50例行单侧腮腺手术的患者,术前及术后30 d进行ENoG及sEMG检测,记录波幅(Am)及运动单元动作电位(MUAP),计算得出神经变性率(DR)和平均发放间隔(IPI),并在同一时间点记录患者的HB分区评价分级。采用SPSS 20.0软件包对数据进行统计学分析。结果:手术后,健、患侧面神经间Am差异有统计学意义(P<0.05)。HB分区评价面神经功能级别越低,DR值越小。在面神经功能评价Ⅳ-Ⅴ级病例中,健、患侧MUAP个数比及IPI参数较术前水平有显著改变(P<0.05)。结论:ENo G的Am和DR参数可作为临床评价面神经功能的客观参考指标,sEMG参数在评价面神经功能领域的应用需进一步研究。
