关节镜下治疗膝关节骨关节炎的疗效随访

摘要：

目的对关节镜手术治疗膝关节骨关节炎的疗效进行随访。方法分析127例因膝关节骨关节炎于温州医科大学附属第一医院行关节镜手术治疗的患者127例临床资料及随访资料,纳入标准如下:1年龄大于50岁;2临床表现及影像学表现支持骨关节炎诊断;3Ahlbck分级为ⅠⅢ级。治疗前后采用Lysholm及国际膝关节评分委员会（International Knee Documenting Committee,IKDC）评分标准评价主观症状。结果最后一次随访时Lysholm、IKDC膝关节功能评分均优于手术前,差异有统计学意义,Lysholm评分及IKDC评分:最后一次随访时及手术前评分分别为87.9±14.0、79.1±13.5以及54.6±12.3、54.9±9.7,差异有统计学意义（P均为0.01）。76%的患者术后膝关节功能为优-良。50～70岁患者术后评分高于术前,差异有统计学意义,Lysholm评分及IKDC评分:最后一次随访时及手术前评分分别为81.3±13.9、80.2±11.9及59.0±10.1、60.6±2.4,差异有统计学意义（P均为0.02）。70岁以上患者术后与术前评分差异无统计学意义,Lysholm评分及IKDC评分:74.5±9.2、67.6±8.6及55.8±9.6、56.3±8.5（P均为0.09）。结论中期随访研究表明关节镜清理术对70岁以下的膝关节骨关节炎可取得较好的疗效。
