体外冲击波对偏瘫患者上肢痉挛的疗效观察

摘要
目的观察低能量体外冲击波对偏瘫患者上肢痉挛的治疗效果。方法将30例伴有上肢痉挛的偏瘫患者分为治疗组和对照组,每组15例。对照组进行综合康复治疗;治疗组在此基础上加用体外冲击波治疗(ESWT),每周1次,共4周。ESWT前和每次ESWT后以及4周后随访时,应用改良Ashworth量表(MAS)和简化Fugl-Meyer评定法(FMA)上肢部分进行评定。结果组内比较:治疗组在每次ESWT后及随访时,MAS、FMA评分均较治疗前改善(P<0.01);对照组MAS、FMA评分与治疗前相比无显著性差异(P>0.05),仅在第4次ESWT后及随访时与治疗前相比有显著性差异(P<0.05)。组间比较:每次ESWT后及随访时,治疗组MAS均较对照组改善(P<0.01),第4次ESWT后及随访时,治疗组FMA较对照组改善(P<0.01)。治疗组均无不良反应发生。结论ESWT对偏瘫患者上肢痉挛具有一定的疗效;4周随访时,疗效持续存在,且安全无副作用。
