复发性髌骨脱位J形征形成的风险因素分析

摘要
目的:分析复发性髌骨脱位患者中髌骨运动轨迹出现J形征的相关风险因素。方法:2014年6月至2015年5月,连续共54例诊断为复发性髌骨脱位患者入选本研究,其中9例患者存在髌骨运动轨迹J形征。全部患者均接受膝关节正、侧位X线片检查,下肢负重位全长X线片检查,髋、膝、踝关节CT检查。测量下肢力线、髌骨高度、髌骨外倾角度和外偏程度、股骨滑车发育情况、胫骨结节相对股骨滑车外偏程度、股骨前倾角以及胫骨外旋角度。对比出现J形征患者与其他患者在以上参数的差异。结果:54例患者中,男性13例,女性41例。平均年龄21.6±6.7岁(16至40岁)。9例患者出现J形征,其中男性1例,女性8例。平均年龄21.9±7.8岁(12至34岁)。Logistic回归分析结果显示,高位髌骨(P<0.01)和伸膝位髌骨外偏(P=0.013<0.05)与J形征存在显著的相关关系。结论:对于复发性髌骨脱位的患者,存在明显高位髌骨或膝关节伸直位髌骨外偏,是导致髌骨运动轨迹出现J形征的风险因素。在治疗这一类型的患者时,需要对高位髌骨和髌骨外偏引起重视并作出适当处理,以确保手术疗效。
