全胸腔镜下非小细胞肺癌根治手术的经验

摘要：

目的探讨非小细胞肺癌全胸腔镜肺叶切除的方法和经验。方法全胸腔镜肺叶切除术16例,肿瘤直径1.3～4.8 cm,其中右上肺叶6例,右中、下肺叶4例,左上肺叶5例,左下肺叶1例。结果手术顺利,无围手术期死亡或严重并发症,无中转开胸。平均手术时间148 min,平均手术出血量152 ml,平均引流量800 ml。术后病理腺癌9例,鳞癌5例,腺鳞癌1例,肺泡细胞癌1例。结论全胸腔镜肺叶切除应选择早期周围型肺癌,手术重点应安全处理肺血管和支气管,超声刀在纵隔淋巴清扫中有独特优势。
