老年急性缺血性脑卒中患者血同型半胱氨酸水平与不良结局的关系

摘要
目的探讨老年急性缺血性脑卒中患者血浆同型半胱氨酸(Hcy)水平与其出院结局不良的关系。方法采用回顾性队列研究的方法,连续性纳入急性缺血性脑卒中住院患者1 688例,收集人口统计学、生活方式和实验室检查结果等资料。结局不良定义为患者出院时改良的Rankin评分(MRS)≥3分,结局良好MRS<3分。采用非条件Logistic回归分析入院时血浆Hcy水平与出院结局不良的关系。结果结局良好组与结局不良组入院时Hcy水平差异无统计学意义(P>0.05)。用三分位数法将入院时Hcy水平分为3组,<12.5μmol/L,12.5~18.6μmol/L,>18.6μmol/L,3组的结局不良发生率分别为29.47%,34.30%,36.23%。经单因素非条件Logistic回归分析结果发现,与入院时<12.5μmol/L组相比,>18.6μmol/L组发生出院结局不良的OR值(95%CI)为1.332(1.012~1.752)。经多因素调整后,同样>18.6μmol/L组增加了急性缺血性脑卒中患者发生出院结局不良的风险,OR值(95%CI)为1.455(1.025~2.131),并且随着Hcy水平的升高发生出院结局不良的风险也在增加(趋势性P<0.05)。结论入院时血浆Hcy水平升高可增加急性缺血性脑卒中患者出院结局不良发生的风险,并存在剂量-反应关系,是独立的危险因素。
