我国台湾面对人口结构老龄化,在老年长期照护实践中开展了多专业团队服务形式。文章针对多专业团队的概念和运作方式及专业人员角色、职责、服务内容和形式进行了阐述,并探讨了多专业团队在实践中的应用及实施个案管理的经验与效果。
