椎间盘内亚甲蓝注射治疗慢性椎间盘源性腰痛的随机临床对照研究

本文是一项关于椎间盘内亚甲蓝(methylene blue,MB)注射用于治疗慢性椎间盘源性腰痛的随机临床对照研究。我们对有临床症状的136例慢性腰痛患者经椎间盘造影后,72例符合纳入标准。所有患者椎间盘源性腰痛的持续时间超过6个月,并且无合并症。36例行椎间盘内亚甲蓝注射,另外36例行安慰剂治疗。主要判断标准包括:疼痛减轻程度(由NSR-101数值等级评定系统评价)以及功能改善程度(由ODI功能障碍指数评估)。24个月随访结果显示两组主要指标有明显差异。亚甲蓝注射组NSR-101、ODI评分分别平均减少52.50、35.88,其满意率为91.6%;而安慰剂治疗对照组分别为0.70%,1.68%和14.3%(分别P<0.001,P<0.001和P<0.001)。椎间盘内注射亚甲蓝组无不良反应及并发症发生。本临床研究表明,椎间盘内亚甲蓝注射是治疗顽固性、致残性椎间盘源性腰痛的一个安全、有效、微创的方法。
