新型抗抑郁药物在母乳喂养中的应用

摘要：

产后抑郁越来越受到人们的关注,其发病率高达10%15%。产后抑郁患者面临哺乳的选择,母乳喂养有诸多的好处,但母亲服用抗抑郁药物会考虑对婴幼儿有何影响,以及哺乳期选用何种药物。本文综述了新型抗抑郁药物氟西汀、舍曲林、帕罗西汀、西酞普兰、艾司西酞普兰、氟伏沙明、文拉法辛、度洛西汀、圣约翰草、米氮平、安非他酮对有关母乳喂养的研究报道。结果显示,舍曲林、西酞普兰和圣约翰草安全性较高,可作为哺乳期用药首选,其他药物则在评估风险利益和告知情况下尝试使用,但由于均缺乏长期效果的评估,则需要谨慎使用,严密监测婴儿情况。SNRI、Nassa等相对较新的药物需更多研究。
