亚低温治疗老年重型颅脑外伤疗效及对免疫细胞影响分析

摘要
目的探讨亚低温治疗老年重型颅脑外伤的疗效及对免疫细胞的影响。方法选择2016年4月到2017年4月上海市第十人民医院接诊的100例老年重型颅脑外伤患者, 通过随机数表法分为观察组(n=50)和对照组(n=50)。观察组采用亚低温治疗, 对照组采用常规治疗。比较两组颅内压(intracranial pressure, ICP)水平、颅脑外伤残疾评定量表(traumatic brain injury disability rating scale, DRS)和格拉斯哥昏迷计分法(glasgow coma score, GCS)评分、术后并发症及免疫细胞CD4+、CD8+、CD4+/CD8+水平。结果治疗后, 观察组的ICP水平在2、4、5 d时均比对照组低[(23.17±4.77)mmHg比(25.83±4.65)mmHg, (15.33±5.15)mmHg比(22.44±4.82)mmHg, (14.21±4.02)mmHg比(20.75±5.35)mmHg](P<0.05);观察组DRS评分在治疗后1周、1月均相较于对照组低[(17.03±2.56)分比, (18.76±3.68)分, (15.27±2.23)分比(17.67±2.85)分](P<0.05);观察组GCS评分在治疗后1周、1月均相较于对照组高[(7.35±2.67)分比(6.12±2.08)分, (10.22±3.73)分比(8.21±3.78)分](P<0.05);观察组血小板减少的发生率相较对照组更高[18%(9/50)比4%(2/50)](P<0.05);观察组免疫细胞CD4+、CD8+、CD4+/CD8+水平均比对照组高(P<0.05)。结论亚低温治疗老年重型颅脑外伤可以使术后神经功能更快恢复, 对患者产生保护作用, 可有效的改善患者的免疫功能, 提高预后能力和生存率, 是一种可靠安全的治疗方法, 值得应用推广。
