摘要
感染后咳嗽是亚急性咳嗽中最常见的原因,一般由病毒感染引起。然而,至今有关的发病机制还不明确,仍然缺乏满意的治疗。咳嗽的发生主要与两种咳嗽感受器有关,感受器的咳嗽反射敏感性(CRS)异常增高是机体咳嗽反应的一个十分重要的原因。由感觉神经末梢(主要是C纤维末梢)释放的神经肽所介导的神经源性炎症可能在CRS改变以及感染后咳嗽发病中起重要作用。病毒感染引起了气道炎症,可以通过多种机制引起神经肽或者神经递质,如P物质和辣椒素受体等释放增多,从而直接和间接地增加咳嗽感受器的反应性。因此,减轻神经源性炎症,阻止感觉神经纤维活化可能成为其未来新的治疗靶点。
