阿托伐他汀对不同程度颈动脉粥样硬化患者外周血T淋巴细胞亚群的影响

摘要
目的观察不同程度的颈动脉粥样硬化(carotid artery sclerosis,CAS)患者外周血T淋巴细胞亚群的分布特点,以及阿托伐他汀对此的影响。方法选择CAS患者212例,按照CAS严重程度分为3组。轻度CAS组73例,中度CAS组96例,重度CAS组43例,每天口服20mg阿托伐他汀,观察治疗前和治疗6个月后的颈动脉内膜中层厚度(IMT)、CAS斑块的数量和稳定性、HDL-C、LDL-C、TG、高敏C反应蛋白(hs-CRP)、CD4T淋巴细胞、CD8T淋巴细胞和CD4/CD8比值变化。结果中度CAS组和重度CAS组治疗后IMT较治疗前明显降低[(1.77±1.26)mmvs(2.23±1.06)mm,(2.15±1.10)mmvs(2.61±1.31)mm,P<0.05]。3组治疗前后IMT和斑块性质比较,差异有统计学意义(P<0.01)。3组治疗前后LDL-C、CD4、CD8、CD4/CD8比较,差异有统计学意义(P<0.05,P<0.01);3组治疗前TG、hs-CRP比较,差异有统计学意义(P<0.05);与治疗前比较,3组治疗后LDL-C、TG、hs-CRP、CD8明显降低,HDL-C、CD4、CD4/CD8明显升高,差异有统计学意义(P<0.05,P<0.01)。结论阿托伐他汀治疗CAS的机制之一是调节T淋巴细胞亚群。
