实验性蛛网膜下腔出血的研究——Ⅰ.动物模型和脑动脉壁的组织学改变

本实验研究发现经枕大池注入自体动脉血可引起狗脑底动脉的急性和慢性痉挛,其发生率分别为83.3%和60%,其程度与动脉周围血液量有密切关系。发生慢性血管痉挛的脑底动脉有形态学改变,其严重性与痉挛程度一致。轻、中度痉挛的动脉在超微结构水平有改变,包括内膜肿胀、紧密连接部分丧失、基底膜增厚、肌细胞间隙增大和核周出现小空泡等。重度痉挛的动脉则在光镜下也可见到明显的内膜退变和肌层环死。作者根据病理学变化推测晚期血管痉挛的发病机理是多因素的,提出脑血管痉挛的处理原则应为预防为主和早期综合治疗。
