早发冠心病多支病变药物洗脱支架植入术后预后影响因素分析

目的:分析影响早发冠心病(PCHD)多支病变患者药物洗脱支架(DES)植入术后预后的主要因素。方法:选择经冠状动脉造影检查确诊冠心病多支病变并首次行DES植入术共516例,根据NECP-ATPⅢ定义分为早发冠心病组187例和晚发冠心病组329例。比较两组存在冠心病危险因素的情况及SNYTAX积分等差异,分析影响DES植入术后预后的危险因素。结果:早发冠心病组男性比例、脑卒中史比例非常显著低于晚发组(P<0.01),肥胖、高脂血症、有冠心病家族史比例显著或非常显著高于晚发组(P<0.05,P<0.01);两组吸烟史、高血压病、糖尿病、陈旧心梗等差异不显著(P>0.05)。早发冠心病组中总胆固醇(TC)、三酰甘油(TG)及低密度脂蛋白胆固醇(LDL-C)水平均显著或非常显著高于晚发组(P<0.05);两组SYNTAX分值差异不显著(P>0.05)。平均随访23个月,两组主要不良心脑血管事件(MACCE)发生率均差异不显著(P>0.05)。COX回归模型分析结果显示,女性及高脂血症病史为PCHD的保护性因素,而吸烟史及冠心病家族史为危险性因素。结论:对于PCHD多支病变并行DES植入术的患者,应积极戒烟、合理膳食,严格控制体重,维持良好的血脂水平,以改善预后,减少不良心血管事件发生。
