住院肿瘤病人发生癌因性疲劳状况的多维度多因素分析

摘要
[目的]了解中国肿瘤人群中住院病人癌因性疲劳的流行情况及其相关影响因素,为有针对性地干预工作提供具体指导。[方法]纳入516例住院肿瘤病人,记录病人一般资料、疾病信息和实验室常规检查结果,填写简明疲劳量表、MDAnderson症状量表、Karnofsky功能状态评分、Zung自评焦虑量表和Zung自评抑郁量表。相关因素分为环境维度、躯体症状维度、心理症状维度和生物医学维度,采用多变量二分类Logistic逐步回归后退法模型进行统计分析。[结果]癌因性疲劳是住院病人最常见的躯体症状,发生率达81.0%,中度以上癌因性疲劳达60.0%。Logistic回归模型分析显示,在4个维度中共有10个因素进入回归模型,环境维度4个因素:年龄、病程、知情状况和工作状态,生物医学维度2个因素:单核细胞计数、白蛋白含量,躯体症状维度2个因素:气短程度和口干程度,心理症状维度2个因素:睡眠不安和瞌睡。多维度Logistic回归模型分析纳入3个变量:单核细胞计数、白蛋白含量和口干程度。[结论]外周血单核细胞计数、白蛋白含量和口干程度是住院肿瘤病人癌因性疲劳发生的独立预测变量,提示全身性慢性炎症过程、相关身心症状、营养状况将是今后癌因性疲劳机制研究和干预实践的主要方向。
