脑卒中患者心肺功能及其影响因素研究

摘要
目的分析脑卒中患者心肺功能及其影响因素。方法选取2016年12月—2018年9月徐州市中心医院康复科收治的脑卒中患者80例,其中4例患者因不能佩戴收集气体的面罩而剔除研究,根据峰值摄氧量(VO2peak)将76例患者分为A组(VO2peak为16~19 ml·kg-1·min-1,n=18)、B组(VO2peak为12~15 ml·kg-1·min-1,n=26)和C组(VO2peak为7~11ml·kg-1·min-1,n=32)。采用自行设计的一般资料调查表收集所有患者一般资料,评定所有患者简化FuglMeyer运动功能评分量表评分、简易精神状态检查量表(MMSE)评分、日常生活活动能力量表(ADL)评分及Berg平衡量表评分;所有患者进行心肺运动试验并记录其VO2peak、峰值摄氧量占预计值的百分比(VO2peak%pred)、无氧阈(AT)、达到AT时摄氧量(VO2AT)及最大运动负荷时呼吸困难和腿部疲劳程度评分;脑卒中患者心肺功能的影响因素分析采用多因素Logistic回归分析。结果 (1) 76例患者均顺利完成心肺运动试验,且未发生任何不良事件。本组患者VO2peak为(12.89±3.42)ml·kg-1·min-1,VO2peak%pred为(46.0±6.1)%,最大运动负荷时呼吸困难、腿部疲劳评分分别为(14.84±0.97)分、(15.55±0.86)分。(2)三组患者文化程度、婚姻状况、卒中类型、偏瘫部位、病程、有无基础疾病、是否使用β-受体阻滞剂及Berg平衡量表评分比较,差异无统计学意义(P>0.05);三组患者年龄、性别、有无不良生活习惯、日常运动习惯、简化Fugl-Meyer运动功能评分量表评分、MMSE评分及ADL评分比较,差异有统计学意义(P<0.05)。(3)多因素Logistic回归分析结果显示,年龄[OR=2.14,95%CI(1.24,4.58)]、性别[OR=1.32,95%CI(1.03,1.94)]、不良生活习惯[OR=2.16,95%CI(1.18,5.17)]、规律的日常运动习惯[OR=0.02,95%CI(0.01,0.20)]、简化Fugl-Meyer运动功能评分量表评分85-95分[OR=0.54,95%CI(0.39,0.83)]、简化Fugl-Meyer运动功能评分量表评分96~99分[OR=0.20,95%CI(0.12,0.37)]、MMSE评分27~28分[OR=0.34,95%CI(0.27,0.66)]及MMSE评分29~30分[OR=0.06,95%CI(0.01,0.19)]是脑卒中患者心肺功能的独立影响因素(P<0.05)。结论脑卒中患者心肺功能明显下降,规律的日常运动习惯、简化Fugl-Meyer运动功能评分量表评分高、MMSE评分高是脑卒中患者心肺功能的保护因素,而年龄大、女性、不良生活习惯是脑卒中患者心肺功能的危险因素。
