应用不同频率、部位腹肌电刺激的反馈式呼吸电刺激训练对慢性阻塞性肺疾病患者膈肌功能和呼吸效率的影响

摘要
目的:探讨应用不同频率、不同腹肌电刺激的反馈式呼吸电刺激训练对腹肌功能、膈肌功能和呼吸效率的影响。方法:募集15例中重度慢性阻塞性肺疾病患者,在同一周内分别随机进行30Hz下腹肌、30Hz腹直肌、50Hz下腹肌、50Hz腹直肌刺激的反馈式呼吸电刺激训练。呼吸电刺激训练主要由膈神经和腹肌电刺激、呼吸反馈、腹式呼吸三个部分组成。膈神经和腹肌电刺激:有两个交替输出的电流通道,A通道输出双向对称方波,频率30Hz,脉宽为0.5ms,刺激膈神经;B通道输出低频调制中频正弦波,载波频率4k Hz,调制波频率在30Hz或50Hz,刺激下腹肌(腹股沟中点上2cm)或脐旁水平2cm处的腹直肌。在平静状态时和训练的第15min分别检测下腹肌肌电、腹横肌厚度、膈肌活动度、潮气量、死腔通气等指标。结果:30Hz、50Hz下腹肌和腹直肌刺激后下腹肌肌电和厚度明显增加,且50Hz下腹肌刺激较其他三组下腹肌肌电和腹横肌增厚率大,且差异显著。30Hz、50Hz下腹肌和腹直肌刺激后膈肌活动度、潮气量明显增加,死腔通气明显减少,且50Hz下腹肌刺激较其他三组刺激膈肌活动度和潮气量大,差异有显著性差异(P<0.05)。结论:反馈式呼吸电刺激训练可以增加腹肌收缩,提高膈肌活动度和呼吸效率。
